4. Fill out page 2 of the form so we know who you are

5. Fill out page 3 so we know where you want your donation sent

6. Please mail or deliver to the the address shown below:

7. Alternately, seal your application in an envelope, write on the envelope "Tierrasanta Foundation", and drop the envelope at the Tierrasanta Branch Library on La Cuenta Drive.

Tickets to be delivered starting this summer (June, probably)

Later this summer we will begin delivery of tickets to your homes (if you live in Tierrasanta) or possibly by US Mail (if you live elsewhere). Please make sure we have your address and a phone number so we can coordinate this for a time that is convenient to you.
